Responsibilities • 50-75% of time will be spent in the skilled nursing unit (Keil Healthcare) • Facilitate and monitor daily activity programming, documenting 1:1 interactions as well as moods and behaviors using CareTracker • Interview new and current residents utilizing the Med-Pass Activities Evaluation form • Review resident medical charts for health and general information to better support resident care through life enrichment activities • Begin learning and using MDS 3.0 for documentation of resident participation, as well as for new admissions, quarterly and annual assignments • Learn how to create an individualized care plan for each resident – addressing needs, wants and goals to better each person’s quality of life • Document, through progress notes, on Medicare residents throughout the lifecycle of their stay (5, 14, 30, 60 and 90 days) as well as long-term care residents (quarterly and annually) • Utilize various programs to create monthly activity calendars to detail resident programming • Additional responsibilities include assisting with daily activities and tasks with assisted living and Pavilion (memory care) • Remaining time will be utilized to understand and facilitate programming with independent residents of the CCRC • Create and host meaningful activities for residents within all levels of care • Modify and adapt activities to resident levels, ensuring complete participation from those residents who have shown an interest • Assist with all outings, whether across campus or in the community • Lead programs such as daily exercise, socials, bingo, bridge, art classes, etc.
